Claiming Scholarship

How to receive your scholarship funds - Complete the Google Form tinyurl.com/claimscholarship

Scholarship funds will be sent directly to the Financial Aid Office at the college/university/program in which you are enrolled. You will have to input the following, please have all information available to you:

1. School Name

2. Financial Aid Office Mailing Address

3. Amount of Funds you want distributed (have certificate from Assist-A-Grad handy)

4. First Name

5. Last Name

6. Student ID Number

7. Term you want the funds applied to (please plan accordingly)

8. High School Graduation Year

9. Proof of registration in the form of a current class schedule (Enrollment verification is NOT accepted) [will be uploaded via the Google Form below]

Please note you have one year until your scholarship expires. Scholarship disbursements need to be requested no later than July 1 the year after you receive the scholarship.
